1. Scope of Services

Smile N Style provides general and specialized dental services in accordance with applicable federal and New Jersey laws. All services are provided by licensed dental professionals.

Dental treatment recommendations are based on individual clinical evaluation and professional judgment. Results cannot be guaranteed.

2. Appointments & Cancellations

• •Appointments must be scheduled in advance.
• •We request at least 24 hours’ notice for cancellations or rescheduling.
• •Failure to provide timely notice or repeated no-shows may result in a cancellation fee or refusal of future appointments, as permitted by New Jersey law.

3. Patient Responsibilities

You agree to:

• •Provide accurate and complete medical, dental, and insurance information
• •Inform us of changes to your health history or insurance coverage
• •Follow recommended treatment plans and post-treatment instructions
• •Treat staff and other patients respectfully

4. Insurance & Payments

• •Patients are responsible for all charges not covered by insurance.
• •Insurance benefits are estimates only and not a guarantee of payment.
• •Payment is due at the time services are rendered unless prior arrangements are made.
• •We accept applicable forms of payment as disclosed by the Practice.

Unpaid balances may be referred to a collection agency in compliance with New Jersey and federal law.

5. Fees & Refunds

• •Fees are based on current procedural codes and may change without notice.
• •Refunds, if applicable, will be handled in accordance with New Jersey regulations and insurance guidelines.

6. Privacy & Protected Health Information

Your personal and health information is handled in accordance with our Notice of Privacy Practices, HIPAA, and New Jersey privacy laws. By using our services, you acknowledge receipt of our Privacy Policy.

7. Website Use Disclaimer (If Applicable)

Any information on our website is provided for general informational purposes only and does not constitute dental or medical advice. Use of our website does not establish a dentist-patient relationship without an in-person consultation.

We are not responsible for technical issues, website interruptions, or third-party links.

8. Limitation of Liability

To the fullest extent permitted by New Jersey law, Smile N Style shall not be liable for any indirect, incidental, or consequential damages arising from services provided, except where prohibited by law.

Nothing in these Terms limits liability for gross negligence, willful misconduct, or violations of applicable law.

9. Termination of Services

We reserve the right to refuse or discontinue treatment in accordance with professional standards, ethical obligations, and New Jersey law, including for non-compliance with treatment plans, payment policies, or abusive behavior.

10. Governing Law

These Terms of Service are governed by the laws of the State of New Jersey, without regard to conflict-of-law principles. Any disputes shall be resolved in the appropriate courts located in New Jersey.

11. Changes to These Terms

Smile N Style reserves the right to update or modify these Terms of Service at any time. Updates will be posted in our office and/or on our website. Continued use of our services constitutes acceptance of the revised terms.
